Half of the comic is dedicated to the girl telling the MC about her experiences with her ex in order to invoke a reaction of jealousy on his part. Netorare is largely a psychological thing, it's not really about the details of whether the MC and the girl are actually in a relationship or whether the MC is actually getting cuckolded - it's about creating a feeling of jealousy in the reader by showing that the MC's love interest isn't "his" or has been "soiled" by having sex with another man.

A good example of this is one of Aiya's works called "Hey, Sensei?" where this shut-in kid has a crush on his teacher. The kid and the teacher are never in a relationship, but the story ends up with the kid finding out that his father has been fucking the shit out of his teacher. The story is undoubtedly netorare if the reader chooses to self-insert as the kid because the love interest ends up being "taken away" despite the two never having a relationship. On the other hand, the story ends up being netori if the reader decides to self-insert as the kid's father.

This particular story tries to create jealousy in the MC (and, by extension, the reader) by telling him in graphic detail how his childhood crush had sex with someone who wasn't him. Hell, she even tries to make him even more jealous by lying to him about the other guy cumming inside her. It was definitely written to appeal to the NTR fandom (the MC even does the classic and hilarious "NTR face" when she tells him she isn't a virgin). However, it is not cheating as the girl was not in a relationship with the MC (or anyone else) at the time that she had sex with the other man.
