Page 16 and 18 aren't straightened properly. But I'd suggest double checking everything.
Edit: I'm saying they weren't really flat when you scanned them. Take a look at the top left of both pages.

Edit: Oh okay... Yeah, every other page seems to be like that... I thought I had two books on it to flatten it out, but maybe that wasn't enough.... I'll probably cut the pages (they were 2x size) and then scan again.
