This is pretty heavy hitting. I think the first three pages really set up the story well. Obviously something happened to the main character to change him from the first page to what we see in the second. The little sister probably treats him like shit because she loves him (not romantically) and just can't stand to see how far he's fallen. He has all this self loathing and instead of dealing with it he rests it all on his little sister's shoulders and plans to get her to kill him.

Yeah, it's ridiculous and could fall under "hentai logic", but I think it adds a deeper sense of regret and anguish between the siblings. She isn't getting raped by some random gross guy. She's getting raped by her gross older brother that she used to have childish romantic feelings for. I'm curious to see where this will end. I wouldn't be surprised if they both end up dead or just the sister considering the title of "Kyoudai Shikkaku" (No longer siblings) is obviously a reference to "Ningen Shikkaku" (No longer human) by Dazai Osamu.
