More seriously, Shinji always falls in love with him after he's been distanced by everyone else, Kaworu is the last ray of hope for Shinji when he's in his biggest moment of despair, somehow I always found it very touching, even when I used to have some reservation about the "sexual implications" of his relation with Kaworu. Actually this doujin isn't explicit at all about the romantic relation, you could even interpret it as a deep friendship if you wish to do so (I think that it's not an accurate interpretation but to each its own).
