Asuna and Kirito do have a child that exists only in the game. As such, she is not a pure NPC entity because she is able to continue to exist and interact without either parent being logged in (as shown in the light novel translations).

Why, because they're not in the game, they did it in the real world. if I can spell my guess. I mean, if the condom get pushed well above the durability limit in game, it'll simply disintegrate instead of getting torn off. Unless if they're playing in another VR game which apparently do exhibits such physics.
