This is some interesting metafiction.

From what I've skimmed, Souta, the MC, has died and was reborn into an isekai identical to a dating sim he had played. He was reborn as the MC in the game! He encounters the kouhai character of the game, Chisa, but he is interested in a different character. In order to be locked into that character's scenario, he's brushes off Chisa harshly. After being rejected by Souta, Chisa coughs of blood and collapses.

Ayaka, another heroine, witnessed what transpired. This heroine is also from another world and revealed what she believes is happening. In dating sims and eroge, once you're locked into a character's route, oftentimes the MC will never encounter the other girls in the game. That is true in this world as well... except, in this world the girls are quite literally "taken out of the game" if not chosen by the MC. After finding out that Ayaka is his real older sister (and the cause of his death!), they come up with the idea to save her: if the MC romances with a heroine, she's still "in-play" and won't be taken out by the world's system. As each of the other heroines discover the truth, they too are willing to do what is needed to survive.
